Medical Compliance
Make sure that your child is healthy and ready to learn by the start of the school year! Proof of required immunizations, physical exams and vision exams are due October 15th of each year. Students that have not met CPS minimum medical compliance criteria by Oct. 15 will receive a school prohibition letter and may be prohibited from school until minimum medical compliance are met.
