Returning Family Information
Due to our remote learning model, we are changing the way that we collect information from returning families. There are forms that required to be turned in annually by Chicago Public Schools. Typically, these forms are completed and turned into the teacher during the first week of school.
There are two options for how to go about completing these forms:

- You can complete the required forms at the Remote Learning Supplies Pick Up events. The dates and times are on the attachment below.
